What you need depends on where you are.
Pre-Seed / Seed
D&O (investors require it), GL (your lease requires it), and basic cyber. Keep it lean — you can scale coverage as you scale the business.
Series A
Add E&O (your customers will require it), upgrade cyber limits, and consider EPLI as you scale hiring. Your first enterprise contract will dictate minimum coverage.
Series B+
Full program: D&O with higher limits, robust E&O, cyber with incident response, EPLI, fiduciary liability, and potentially IP/media liability. Your board and customers both demand comprehensive coverage.
Bootstrapped / Profitable
